Introduction

Physique bodyworks values your privacy, however needs to collect certain information from you and needs your authorisation to retain this information.  This document explains what and why it is collected, how long it is kept and your rights about it going forward.

Section 1: Your Contact Details and Personal Health Information

Lawful Basis for collecting this information: Contract

What information is needed? : Your name, address and postcode, your preferred contact telephone number, and optionally your email address.  Also your age, information about your health and lifestyle history and any medical conditions you may have.

Why is it needed? : This information is needed for legal and insurance purposes, for managing your appointments and to ensure that treatments offered are appropriate to your health conditions.  We may also email you self-care advice between treatments.

How long will this information be retained? : Your personal information and treatment record is kept for a minimum of Eight Years* from the date of your last treatment session for both legal and insurance purposes.  After this period the information will be destroyed without the need for any action on your part. (*- or until 25^th birthday if a minor at the time of treatment)

How will this information be stored? : This information can be stored securely on both paper based record cards and an industry approved password protected secure database.

Will this information be shared with any other parties? : No.  With the possible exception of any future legal requirements your information will not be shared.

Section 2: Marketing

From time to time Physique bodyworks may wish to contact you regarding services available or for promotional reasons.  You do not have to agree to this in order to receive treatment.  If you agree to receive occasional marketing information, please tick the relevant boxes below:

Section 3: Your rights

Your right to Deletion of Information / Your Right to Be Forgotten: For legal and insurance reasons your treatment record must be retained for a period of Eight Years from the last treatment.  During the intervening period your record will be securely archived and will not be easily accessible.
